In the event of an emergency, I allow Pillars Sports Academy to seek medical treatment for my child if I am not available to provide consent. I agree to cover any medical costs.

I agree that photographs or videos of my child may be taken and used in social media, promotions, or any other Academy materials.

My child agrees to follow the rules set by Pillars Sport Academy, and I understand that breaking the rules may lead to suspension or removal from the program without a refund.
